Exploring Self-Health Management Models for Residents Based on Digital Technologies

To address the challenges of low health literacy, weak self-health management awareness, a large number of chronic disease patients, and heavy disease burden among community residents, Changxing County has adopted a health outcome-oriented approach. A digital information platform has been established to integrate 21 daily health activities. These activities are assigned points based on their importance, and a point-based reward system has been set up, allowing residents to exchange points for health gifts. This initiative aims to encourage active participation in health activities, gradually improve community health literacy, and cultivate healthy behavior habits. Ultimately, the goal is to prevent and control the occurrence and development of non-communicable diseases. Additionally, real-time collection of health activity data provides support for the integration of medical care and prevention, as well as chronic disease management. The specific implementation path includes: 1.Conducting baseline surveys and collecting population health data. 2.Establishing a working mechanism led by the government, assisted by various departments, and involving the whole society. 3.Building a digital information platform to guide residents in participating in the point-based reward system for health gifts. 4.Organizing various health activities within the community and providing personalized health services to residents.
